We investigate to take lesson learned, implement preventive measures, and to prevent recurrence of adverse events. Investigating an adverse event is like peeling an onion. We start with the direct causes that are the most visible. To get to the bottom of the events, we must uncover "layer upon layer" to discover the causes behind. Technological development and new organizational structures lead to an increase in complexity, and it becomes more difficult to understand the connection between an incident and the underlying causes of an incident. This course presents method, organisation, and process for conducting investigations. The course can also be offered in combination with a classroom session.

COURSE DESCRIPTION

In a risk assessment,  potential events and consequences are considered. In this course, we focus on unwanted events such as failures and accidents. This could be, for example, a gas leak or afire at an oil platform, that equipment stops working during an operation in a hospital, or that an accident occurs during the execution of a work task. The risk analysis process consists of three phases:

1) Planning

2) Risk assessment

3) Risk management

This course shows how these phases can be planned and implemented in practice. There are many guidelines and standards on how a risk assessmentcan be carried out, and many are adapted to particular industries and professional areas. This course presents a general approach that can be used in most industries.

•Risk assessment and risk management:Description of the steps in a risk assessment, what is important to look out for in the various steps, and how the risk assessment can be applied in risk management.

An introduction is given to some of these; Hazid/ course risk analysis, SJA, HAZOP, FMEA and QRA. It’s crucial to choose a method that is well adapted to the problem you are looking at.

Forskriften krever opplæring for ulike stillashøyder:
